Community Themes and Strengths Assessment Survey

This survey is being conducted by a Southeast Ohio Regional Core Group, consisting of local Health Departments and Holzer Health System representatives. This group is working toward completing Community Health Assessments for each county in our region to identify key health needs and issues through systematic, comprehensive data collection and analysis. The findings from these health assessments will guide the group in completing Community Health Improvement Plans. A Community Health Improvement Plan (or CHIP) is a long-term, systematic effort to address public health problems based on the results of community health assessment activities and the community health improvement process. A plan is typically updated every three to five years.

The primary goal of this Regional Core Group is to improve the health of the populations within the Southeast Ohio region. The purpose of this survey is to gain a better understanding of the barriers to healthy living that are present within your community. This short survey should take no more than 5 minutes of your time and your responses will be kept completely anonymous.
