Holzer Care Management Testimonial
After two hospital stays, Jeff Gillum was a candidate for Holzer’s Care Management Program, a service that assists with healthcare needs through a designated Holzer employee. “Caitlin has benefitted my health greatly,” shared Jeff. “She was able to help me get appointments made and answers my questions quickly.”
Jeff has worked closely with several Holzer providers, including Drs. Huntley, Bradley, Rajan, and others to ensure that his health and quality of life are the best they can be. “The communication between providers and service lines has been excellent,” shared Jeff’s wife, Jeanne. “With the help of Care Management, I have been able to care for him at home and keep him home. We’ve had phone calls from Dr. Huntley at 7pm just to make sure everything is going well. That means so much to us.”
Holzer Care Management Program is a liaison between primary care and medical home initiatives. This allows patients who have a complex care plan to receive comprehensive care. This is accomplished through a team effort between physicians, clinicians, patients, and caregivers, working together to help patients recover and have the best quality of life possible.

Jeff and Jeanne live in Jackson, Ohio. Jeff enjoys playing guitar, gardening, and spending time with his granddaughters.
