What is Health Equity?

Holzer is pleased to join health systems across the nation participating in the Framework for Health Equity established by the Centers for Medicare and Medicaid Services (CMS). We are committed to helping all individuals in our communities attain the highest level of health, and we believe that the opportunity to attain optimal health benefits our entire communities by using our limited health care resources more efficiently.

Our Mission drives us to provide Every patient with a Friendly Visit and Excellent Care. It is also our aim that Every individual has equitable access to care. We believe all persons deserve the chance to live their healthiest life, and we are committed to addressing hidden barriers to care in the communities we serve.

Our Strategic Plan

We demonstrate this commitment by including health equity into our strategic planning efforts and dedicating organizational resources. Our Health Equity Strategic Plan focuses on the following:

  • Leadership and Governance
  • Workforce Development
  • Data Collection, Standardization, and Accuracy
  • Health Disparities Identification and Management
  • Awareness and Community Agencies Collaborations

The reach of our efforts extends beyond the walls of our health system. We will continuously collaborate with community-based organizations and local officials to develop shared goals, establish discrete actions, and align resources to meet the unmet needs within our communities. We will meet on a reoccurring basis to share updates, monitor progress, and adjust efforts whereas necessary. Further, we are committed to expanding our Patient Advisory and Partnership program to invite diverse patient populations to lend their voice and together, reduce disparities of care.

We sensitively collect demographic information such as race, ethnicity, spoken language, and social determinants to understand any health disparities amongst our patient populations. If any disparities are identified, we create quality improvement action plans to eliminate disparity gaps and continuously monitor our progress. 

As we move forward, our primary goal remains to deliver a healthcare experience that is safe, high-quality, patient-centered, and equitable to all we serve.
